Payment arrangements must be submitted and approved prior to disconnection of service.
Failure to make any of these payments by the specified due dates, will void this agreement and result in the cancellation of any remaining installments. No further extensions will be granted, and any remaining balance will be due in full. I understand that upon approval of the request I am agreeing to make these payments, in the specified amounts, no later than the date(s) indicated. I am required to keep any new billings to my account current and paid before they become past due.
